Program of All-Inclusive Care for the Elderly (PACE)

The PACE organization’s clinical team (IDT) will determine which services will best fit your assessed needs. PACE also includes other services determined necessary by the health professional’s team to improve and maintain your health.

The Interdisciplinary Provider Team (IDT) will provide the services. The IDT meets to ensure that your comprehensive medical and social needs are met. Teams typically meet daily to discuss your status.

Minimally, the team is composed of a:

primary care physician nurse social worker physical therapist occupational therapist recreational therapist or activity coordinator

dietitian PACE center supervisor home care liaison personal care attendants transportation \ driver

The Focus is on the Consumer: A team of health care professionals will help you make decisions to ensure a good quality of life. You and your family will play an active role as the team develops and updates the plan of care and goals in your program.

PACE Covers Prescriptions: PACE organizations offer Medicare prescription drug coverage (Part D).

PACE Supports Family Caregivers : PACE provides care giving training, support groups and respite care to help families keep their loved ones in the community.

Preventative Care is Covered and Encouraged: Every PACE organization is focused on helping older adults live in the community for as long as possible. To meet this goal, PACE organizations focus on preventative care.

PACE Provides Medical Transportation: You may be able to get transportation to some medical appointments in the community. The PACE site must provide transportation to medical appointments.

PACE uses Medicare and Medicaid funds to cover all medically necessary care and services. You can have either Medicare or Medicaid, or both, to join PACE. They can also pay for PACE privately, if you do not have Medicare/Medicaid.

PACE History

Kansas's first PACE provider, Via Christi Hope began in 2002. Via Christi HOPE serves Sedgwick county.

Midland Care Services started PACE services in 2007 and serve Douglas, Jackson, Jefferson, Lyon, Marshall, Nemaha, Osage, Shawnee, Pottawatomie and Wabaunsee counties.

Bluestem Communities started PACE services in 2015 and serves Lincoln, Ottawa, Ellsworth, Saline, Dickinson, Rice, McPherson, Marion, Reno and Harvey counties. Contact Bluestem at 1-844-588-7223.
