If your child meets the program criteria for the HCBS SED Program, the Community Mental Health Center (CMHC) in conjunction with the youth and their family will create an interim plan of care with 14 days of meeting eligibility. The interim plan serves as a guide for services until your child is assigned to a Managed Care Organization (MCO). The CMHC will also assist you in completing the Medicaid application (if they do not have a Medicaid card).
Once a MCO has been assigned, a meeting will be scheduled with 30 days. The MCO, CMHC, child and anyone the participant or you identifies will participate in the meeting. The goal of the meeting is for the MCO to create the official plan of care reflecting which waiver services the participant will utilize.
