PEAK: Quality Improvement through Person-Centered Care

The Kansas Department for Aging and Disability Services (KDADS) is committed to ensuring high-quality services for Kansas nursing home residents. For more than 20 years, KDADS has recognized nursing homes for successfully implementing person-directed practice through its PEAK program. From the beginning of PEAK, KDADS pursued two primary goals:

  1. To award and recognize homes accomplishing significant culture change; and
  2. To educate others about the culture change movement and the accomplishments being made in Kansas.

Beginning in 2012, PEAK expanded from a recognition program to a pay-for-performance Medicaid program to increase the adoption rate of person-centered care practices in Kansas nursing homes. The program had an overwhelming response, with 125 homes enrolling in PEAK 2.0 in the first year. It was then that KDADS arranged a partnership with the Kansas State University (KSU) Center on Aging to administer the program.

KDADS continues to manage the payment of incentives earned, and the KSU Center on Aging handles program administration, including application, education, and evaluation. Please visit the KSU Center on Aging website to learn more about the program.

Update on SFY25-26 PEAK Advisory Members

The PEAK Advisory Board for SFY 25-26 has many fresh, wise voices joining this year. The summer months were spent planning the 2025 Person-Centered Care Conference, held on October 15th and 16th, 2025. The rest of the program year will focus on initiatives to improve the PEAK program, based on results from the survey performed by last year’s advisory team.


PEAK Survey Results and Action Steps

In January 2025, the PEAK Advisory Team distributed a survey to both PEAK and non-PEAK homes to identify successes and barriers related to participation in the PEAK program. After reviewing the responses, PEAK leadership met to discuss the feedback and develop strategies to address areas of concern. This document highlights the survey results and outlines the action steps the PEAK team has begun implementing in response to what was shared.

2025 Person-Centered Care Conference

The 2025 Person-Centered Care Conference was held October 15–16 at Kansas State University’s Manhattan campus. Over two days, providers, leaders, and stakeholders from across Kansas came together to learn, celebrate, and collaborate in support of person-directed care. Both PEAK and non-PEAK homes participated, reflecting a shared commitment to enhancing quality of life and honoring the voices of those who live and work in our nursing homes.
