Physical Disability (PD)
The Physical Disability (PD) waiver serves individuals 16 to 64 years of age who meet the criteria for nursing facility placement due to their physical disability, who are determined disabled by social security standards, and who are Medicaid eligible.

Eligibility
To be eligible for the PD waiver, an individual must meet the following criteria:
- Be at least 16 years of age and no older than 64 years;
- Be determined disabled by the Social Security Administration;
- Need assistance to perform activities of daily living;
- Meet the Medicaid nursing facility threshold score;
- Individuals who have been determined eligible for the Intellectual/Developmental Disability Waiver (I/DD) are not eligible for the Physical Disability waiver;
- Be financially eligible for Medicaid.
Enrollment
To apply for the Physical Disability Waiver, please start the process by contacting the Aging and Disability Resource Center (ADRC). The ADRC will provide you with Options Counseling and refer you to the Assessing Entity to conduct a PD Functional Eligibility Assessment to determine functional eligibility for the PD waiver. Please contact the ADRC at 1-855-200-2372.

Administrative Case Managers (ACM) help Kansans who have met the Medicaid waiver threshold criteria for Home and Community Based Services (HCBS) with obtaining and maintaining Medicaid financial eligibility. ACMs can also help obtain medical documentation required for HCBS waiver eligibility. Administrative Case Managers can explain benefits, eligibility requirements, and provide assistance in filling out applications and collecting documentation. You may receive assistance for one or more of the following:
