Institutional Transition

An Institutional Transitions Program (ITP) and Money Follows the Person (MFP) are the processes to help residents of qualified institutional settings move back into the community and provide them with support to maintain their community tenure.  

Money follows the Person will restart in the State of Kansas July 1, 2026. 

Basic eligibility for ITP and MFP are as follows:

Eligibility:

  • Individual is a current resident in a Nursing Facility, State Hospital, Nursing Facility for Mental health, Brain Injury Residential Facility, Intermediate Care Facility-Individuals with Intellectual Disabilities (ICF-IDD), or Psychiatric Residential Treatment Facility (PRTF) with a minimum stay of ninety (60) consecutive days, and 
  • Individual has current Medicaid eligibility, and 
  • Individual meets HCBS waiver eligibility for the FE, PD, TBI or I/DD waiver, and 
  • Individual has an interest in moving back into the community.

Institutional Transitions are available for four very distinct populations, the descriptions below are based on the Federal Home and Community Based Services (HCBS) Program descriptions:

HCBS Waiver Waitlist

  • Individuals completing an Institutional Transition/MFP application prior to discharge will bypass any HCBS waiver waitlists.

Independent home or apartment

Group home

Assisted living complex

Homes Plus

Move in with family or friends

Home or apartment a family member owns or leases for the applicant

Cohabitation with a family member that owns or leases a home or apartment the applicant will live with

A group housing arrangement with 4 or fewer unrelated people

Assisted living complex where the applicant leases their own apartment studio or 1 or more standard bedroom apartment

Post transition acclimation services
