CCBHC Information for Providers

To begin the application process, email an inquiry with organization name and list of current services to the CCBHC inbox: [email protected].

KDADS will send a link to complete the Needs Assessment.

KDADS will send the CCBHC Application and CCBHC Certification Checklist to the organization. Submit the CCBHC Application form along with all supporting documents to your encrypted folder in the shared drive.

The CMHC will meet with the CCBHC Rates Administrator to discuss the proper steps for completing a Cost Report. This Cost Report must be completed  prior to submission of an application.

Submit the Cost Report.

KDADS will score the submitted materials. If a score of 90 percent or above is achieved, KDADS will send a Provisional Certification Letter to the organization. If a score is below 90 percent after the first review, KDADS will provide feedback and the organization will have two weeks to submit corrections.

After passing the initial review, the CMHC must submit a copy of the Provisional Certification Letter to KMAP to apply for a CCBHC Provider Number. 

KDADS will schedule an onsite “mock” survey to determine the readiness of the CCBHC. KDADS will interview a group of at least six managerial staff (i.e. team leads, managers) and a separate group of at least eight direct care staff members (i.e. case managers, therapists). These supervisors and staff should be representative of the various programs and locations within the organization. Additionally, KDADS will evaluate for fidelity to the CCBHC Model including, but not limited to, fidelity of EBPs, appropriate staffing, CCBHC culture, and adherence to same-day service delivery.

The provisionally certified CCBHC will provide an implementation plan to KDADS that addresses how the CCBHC will correct and fill any gaps identified during the “mock” survey.

KDADS will review and approve the Implementation Plan. This plan, along with fidelity to models, will be the basis of the full Certification site review.

Upon approval of the implementation plan, a call will be set up with executive leadership to discuss certification status. If the organization is certified, a signed letter, certificate and Memorandum of Understanding will be sent to the organization. If the organization is not certified, a detailed list of gaps will be provided and a reassessment scheduled to occur within 3 months.

Certification is active until the center’s next CMHC recertification, after which, the organization may be eligible for recertification every CMHC recertification cycle.
